Process to Add Name in UP Ration Card
To add a name to UP ration card, first get the application form for addition of member. You can download this form from here:
- After getting the name addition form, fill it properly. Such as – ration card number, applicant name, Aadhaar card number, applicant mobile number, full address etc.
- Reason for addition of new member also to be written in the form. Such as – new baby born in the house, newly married, member name not added in ration card etc.

- Then submit the application form to the concerned officer / employee in Food and Logistics Department office.
